Fees and Insurance
At Mobilize Physical Therapy all of your appointments are scheduled for 55 minutes with Doctors of Physical Therapy. All physical therapy services are billed by time according to American Medical Association guidelines. For each physical therapy service provided we bill insurance companies $60.50 for every 8-22 minutes of treatment for a standard, follow-up appointment. In general, multiple procedures are done during a standard appointment and therefore multiple procedure codes are billed. For initial evaluation appointments we charge insurance companies $181.50 and often additional charges are billed depending on the treatment provided at the initial visit. This results in a charge of approximately $242-302.50 per standard follow-up visit, and $363 for initial evaluations. Insurance companies pay us a percentage of this cost as part of our contract.
We are Preferred Providers (In-Network) with the following insurances:
- Blue Cross
- Blue Shield
- First Choice
- Kaiser Permanente (KFHP of WA Options)
- Motor Vehicle Insurances
- Labor and Industries (L&I)
We offer a cash pay discount to patients that pay at the time of their visit. If you have an out of network insurance, we will happily provide you with a completed claim form to submit to your insurance for reimbursement.
As a service, we try to verify your insurance benefits prior to your first appointment.
We do not accept Medicaid Insurance.
